The importance of the cultural dimension of food in understanding the lack of adherence to diet regimens among Mayan people with diabetes

Objective:
To understand non-adherence to medically recommended diets among Mayans with diabetes.
Design:Using partially sequential mixed methods, questionnaires, semi-structured brief and in-depth interviews were applied. Questionnaire data were analysed with Pearson’s χ2 and Student’s t tests and qualitative interviews with grounded theory microanalysis.
Setting:Rural, predominantly Mayan communities in Chiapas, Quintana Roo and Yucatan, Mexico, 2008–2012.
Participants:Purposive sample of Mayans with type 2 diabetes; using public health care; 168 women and twenty-seven men; age 21–50+ years.
Results:Participants understood diabetes as caused by negative emotions, divine punishment, revenge via spells, chemicals in food and high sugar/fat consumption. Eliminating corn, pork, sugary beverages and inexpensive industrialized foods was perceived as difficult or impossible. More Mayans reporting not understanding physician instructions (30 v. 18 %) reported difficulty reducing red meat consumption (P = 0·051). Non-adherence was influenced by lack of patient–provider shared knowledge and medical recommendations misaligned with local culture. Men whose wives prepared their meals, women who liked vegetables and young adults whose mothers prepared their meals reported greater adherence to dietary recommendations. Partial adherents said it made life tolerable and those making no physician-recommended dietary changes considered them too restrictive (they meant ‘starving to death’). Over half (57 %) of participants reported non-adherence; the two principal reasons were dislike of recommended foods (52·5 %) and high cost (26·2 %).
Conclusions:Adherence to dietary regimens in diabetes treatment is largely related to social and cultural issues. Taking cultural diversity, food preferences, local food availability and poverty into consideration is essential when developing health-promotion activities related to diabetes.

The seventh annual Teaching and Learning Conference (TLC) was held in Philadelphia, Pennsylvania, from February 5 to 7, 2010, with 224 attendees onsite. The theme for the meeting was “Advancing Excellence in Teaching Political Science.” Using the working-group model, the TLC track format encourages in-depth discussion and debate on research dealing with the scholarship of teaching and learning.

The tin–silver–copper eutectic is a three-phase eutectic consisting of Ag3Sn plates and Cu6Sn5 rods in a (Sn) matrix. It was thought that the two phases would coarsen independently. Directionally solidified ternary eutectic and binary eutectic samples were isothermally annealed. Coarsening of the Cu6Sn5 rods in the binary and ternary eutectics had activation energies of 73 ± 3 and 82 ± 4 kJmol-1, respectively. This indicates volume copper diffusion is the rate controlling mechanism in both. The Ag3Sn plates break down and then coarsen. The activation energies for the plate breakdown process were 35 ± 3 and 38 ± 3 kJmol-1 for the binary and ternary samples respectively. This indicates that tin diffusion along the Ag3Sn/(Sn) interfaces is the most likely the rate-controlling mechanism. The rate-controlling mechanisms for Cu6Sn5 coarsening and Ag3Sn plate breakdown are the same in the ternary and binary systems, indicating that the phases evolve microstructurally independently of one another in the ternary eutectic.

Coarsening of the ternary eutectic in cast Sn–Ag–Cu lead-free solder alloys was investigated. The process was found to follow r3 ∝ t kinetics where r is the rod radius of the dispersed phase and t is time. The effective activation energy for the process is 69 ± 5 kJmol-1. The two types of intermetallic rods, Cu6Sn5 and Ag3Sn, in the eutectic structure coarsen at different rates, with each having a different rate-controlling mechanism. The overall coarsening kinetics for the Sn–Ag–Cu ternary eutectic is significantly slower than that found for the Pb-Sn eutectic, which has implications for long-term reliability of Sn–Ag–Cu solder joints.
Examination of Trichoderma phylogenies derived from ribosomal DNA sequence data

Ribosomal DNA sequences were assessed for their usefulness in distinguishing among Trichoderma isolates and for their robustness in resolving their phylogenetic relationships. DNA sequences from the D2 region of the 28S rRNA gene were determined for 50 Trichoderma isolates representing seven species. Eight distinct sequence types existed, and were mostly consistent with groupings based on morphology. Sequence variability within the D2 region alone was not sufficient to provide a reliable phylogeny. Sequences from the ITS1, 5.8S and ITS2 regions were subsequently determined for 18 of the isolates. Eight distinct ITS sequence types were detected among these 18 isolates. The ITS sequence types were generally consistent with morphology, ITS1 sequence data supported the identification of the Th3 T. harzianum group of Muthumeenakshi et al. (1994) as T. atroviride. The data also confirmed that the biocontrol strains of this study were different from those causing disease problems in the mushroom industry in Europe and North America. Results from the phylogenetic analysis stress the importance of testing the robustness of data used to predict phylogenies. Two ITS sequence data sets for the same group of isolates produced significantly different phylogenies. Congruence analysis detected that T. inhamatum (GJS90-90) was corrupting tree topologies and ‘first order pruning’ was performed by removing its sequence from the two ITS data sets. Subsequent differences in the topologies of pruned ITS1 and ITS2 trees were attributed to a lack of phylogenetic information in the ITS2 sequence region. Although ITS sequences successfully differentiated among morphologically distinct isolates within Trichoderma, it did not provide a sufficient phylogenetic signal to resolve all of their relationships.
Rumen bacterial protein synthesis and the proportion of dietary protein escaping degradation in the rumen of sheep

1. The effect of supplementing barley diets with urea (U), extracted, decorticated groundnut meal (GNM) or Peruvian fish meal (PFM) on rumen bacterial protein synthesis and the proportion of undegraded food protein passing to the duodenum of sheep has been examined.
2. Three wethers were given isonitrogenous, isoenergetic diets containing (g/kg dry matter (DM)): U 20, GNM 106 or PFM 78, the crude protein (nitrogen × 6.25) contents being 139, 145 and 148 respectively. The sheep were fed hourly, the mean daily intake of DM being 0.634 kg.
3. Rumen bacterial protein synthesis was determined using 35S and diaminopimelic acid (DAPA) as bacterial markers and polyethylene glycol (PEG) and chromic oxide as markers of digesta flow. Rumen volatile fatty acid (VFA) production rate was determined by a continuous infusion of [I-14C]acetate. 4. and DAPA gave similar estimates of the proportion of bacterial N in the trichloroacetic acid- precipitable nitrogen of the rumen digesta, the mean value being 0.86. The VFA production rate did not vary significantly between diets, the mean being 5.8 mol/24 h. The flow of bacterial N from the rumen was calculated from the PEG and Cr2O3, estimates of flow and the 35S and DAPA estimates of the proportion of bacterial N in the rumen. and DAPA gave similar values (mean 12.5 g/24 h) and Cr2O3, gave a slightly lower value (11.5 g/24 h) than PEG (13.5 g/24 h). Dietary effects, averaged over the four methods, were not significant; the values were 13.0, 13.4 and I 1–0 g/24 h for the U, GNM and PFM diets respectively.
5. Duodenal samples were taken from two 12h continuous collections from re-entrant cannulas and the DM flow adjusted to total recovery of Cr2O3,. The mean recovery of Cr203 at the duodenum was 0.798. The rates of flow of DM were 0.296,0.311 and 0.334 kg/24 h and of non-ammonia-N (NAN) 13.5, 15.2 and 15.4 g/24 h on the U, GNM and PFM diets respectively.
6. The concentrationsof the essential amino acids in duodenal digestaweregenerally higher with the PFM diet than with either of the other two diets. The flow of most amino acids through the duodenum was generally higher on the PFM and GNM diets than on the U diet.
7. The energetic efficiency of bacterial protein synthesis was calculated to be 2.1 g bacterial N/mol VFA or 28 g bacterial N/kg organic matter fermented in the rumen.
8. From the estimates of bacterial N flow from the rumen and NAN flow through the duodenum it was calculated that 0.22 and 0.69 of the supplemental N from GNM and PFM respectively passed through the rumen undegraded.
Determination of nitrogen requirement for microbial growth from the effect of urea supplementation of a low N diet on abomasal N flow and N recycling in wethers and lambs

1. Plasma urea entry rate, urinary urea excretion and, by difference, urea recycling in the body, together with the flow of non-ammonia N through the abomasum and digestion of dry matter (dm) before the abomasum were determined in both wethers and lambs receiving cereal-starch diets supplemented with urea to give 60–120 g crude protein (N × 6.25)/kg dm.
2. Lambs excreted less urea in urine than wethers given the same diet.
3. Relationships between plasma urea entry rate or urine urea excretion rate and plasma urea concentration were different for lambs compared to wethers suggesting greater conser vation of body N by renal control in lambs.
4. Recycling of urea was not related to plasma urea concentration in wethers but was related exponentially in lambs, suggesting recycling is controlled rather than the result of simple diffusion from the blood to the gastro-intestinal tract.
5. Abomasal non-ammonia-N flow was similar for wethers and lambs and increased linearly with urea supplementation.
6. dm digestion prior to the abomasum was not significantly altered, although there was a tendency for decreased digestion of the basal diet given to lambs.
7. Maximum microbial N flow to the abomasum was estimated as 30 g N/kg organic matter (OM) fermented in the rumen.
8. This work and the literature reviewed suggested maximum net microbial production can be obtained when the diet supplies an amount of fermentable N equal to the microbial N output. It is calculated the diet should supply approximately 26 g fermentable N/kg digestible OM or 1.8 g fermentable N/MJ metabolizable energy. This corresponds to a fermentable crude protein supply varying from 65 to 130 g/kg DM as digestible OM content increases from 400 to 800 g/kg DM.
